All-on-4 is a full-arch dental implant technique where an entire set of fixed teeth (typically 10 to 14) is supported by just four titanium implants placed in the jawbone. The name literally describes the concept: all teeth on four implants.
What makes this technique revolutionary is the angulation of the posterior implants. Two implants are placed vertically at the front of the jaw, while two are angled at approximately 45 degrees towards the back. This angulation maximises bone contact, engages stronger bone at the front of the jaw, and allows the bridge to extend further back without needing implants in areas where bone is often insufficient.
The result is a fixed, non-removable arch of teeth that restores approximately 90% of natural biting force — a dramatic improvement over conventional dentures which restore only about 25%.
All-on-4 is suitable for patients who are missing all teeth in one or both jaws, have multiple failing teeth that require extraction, or who currently wear dentures and want a permanent fixed solution. Good candidates are in reasonable general health and have sufficient bone at the front of the jaw for implant placement.
Importantly, patients who have been told they need extensive bone grafting for conventional implants may still be candidates for All-on-4 — because the angled implant technique is specifically designed to work with reduced bone volume in the posterior jaw.

Traditional dentures rest on the gums and are held in place by suction or adhesive. They can slip during eating and speaking, restrict what you can eat, and contribute to jawbone loss over time because they do not stimulate the bone the way tooth roots do.
All-on-4 provides a fixed set of teeth anchored directly to the jawbone. You never remove them. They restore approximately 90% of natural biting force (compared to 25% for dentures), allow you to eat virtually any food, and preserve jawbone health by providing mechanical stimulation similar to natural tooth roots.
The trade-off is cost and surgery. Dentures are far less expensive and require no surgery. All-on-4 is a significant investment but delivers a fundamentally different quality of life — fixed teeth that feel, function, and look like your own.
Replacing every missing tooth with its own implant (8 to 14 per arch) provides the most natural tooth-by-tooth result and slightly higher biting force (~95% vs ~90%). However, it requires significantly more implants, more surgery, more healing time, and costs $30,000 to $60,000 or more per arch.
All-on-4 achieves a comparable functional and aesthetic result using just 4 implants per arch, at roughly one-third to one-half the cost of individual implants. It also often avoids bone grafting, which individual implants frequently require in areas of bone loss.
For most patients missing a full arch, All-on-4 represents the best balance of cost, treatment time, surgical complexity, and clinical outcome. Individual implants may be preferable when only a few teeth are missing, or when a patient specifically wants the tooth-by-tooth approach despite the higher cost.
Days 1 to 3: Swelling and mild discomfort are normal. Take prescribed pain medication and antibiotics. Apply ice packs to reduce swelling. Eat only soft, cool foods (soups, yoghurt, smoothies). Avoid hot drinks and straws.
Week 1: Most patients feel significantly better by day 5 to 7. Continue soft diet and gentle oral hygiene.
Week 2: Return for a review appointment. Sutures are checked and removed. Gradually introduce firmer foods but avoid hard or crunchy items.

The bridge component (the visible teeth) will eventually need refurbishment or replacement due to normal wear from chewing. How long your bridge lasts depends on factors like oral hygiene, whether you grind your teeth (a night guard can help), your diet, and how regularly you attend professional cleans.
